YOUR Rights and Responsibilities

You or your family are responsible for:

  1. maintaining contact with a community physician

  2. being willing to participate and interact in programs that will involve others of different ages, gender, or culture

  3. treating others, including staff and fellow participants, with respect

  4. attending regularly when scheduled, and letting us know as soon as possible when you cannot attend due to illness, vacation, or appointments

  5. arranging reliable transportation to and from the program and paying any fees involved with this

  6. paying the bill of $10 per visit, which is billed at month end

  7. letting staff know if you have concerns or questions

  8. informing staff of changes in contact information or medical status

  9. providing us with a current medication list

  10. bringing any supplies you might need, eg: oxygen, incontinence products, EpiPen, insulin supplies

As your partner in health, our Day Program is committed to:

  1. treat you, your property, and your caregivers with courtesy and respect

  2. promote your optimal level of independence

  3. develop strategies with you to maintain or improve our functions as long as possible

  4. provide support, respite, and education to your family or caregivers

  5. create opportunities for appropriate recreation participation and social interaction

  6. offer a nutritious lunch on your attendance day

  7. uphold a “zero tolerance of abuse” policy

  8. protect your information according to the Health Information Act

  9. communicate your progress or concerns as deemed necessary to Home Care

  10. provide you with community resource information

  11. wear name tags so you know who is caring for you at the program

  12. provide you with an explanation of your account when requested

  13. provide a safe, clean, and comfortable environment
