Program Description
Leisure Days Adult Day Support Services provide structured recreation, socialization, education, exercises, and a nutritional meal to dependent adults who live in the community and whose caregivers need respite

Goals and Objectives
The Day Program Goals support clients and family/caregivers in maintaining and promoting a state of wellness, including mental, physical, and emotional health and the prevention of disease and injury. They are:
To improve and maintain individual levels of wellness by focusing on physical, social, mental, emotional, and spiritual functioning.
To provide services which assist individuals to maintain their independence for as long as possible. To encourage, support, and maximize client’s independence and wellbeing.
To provide leisure opportunities and health education; to provide an opportunity for the client to be engaged in his/her own community.
To support clients and family/caregivers so they can continue to live in their own homes
To provide services, within the continuum of continuing care, which complement and maximize the use of Homecare and other community service providers.
To assist clients/family/caregivers in adjusting to changes in life circumstances.
To assist family/caregivers in the maintenance of his/her own physical, mental, emotional, and spiritual strength.
To delay admission and or ease the transition of a client to a supportive living environment.
To facilitate discharge from long term care/rehabilitation/acute care hospitals.
