Program Description

Leisure Days Adult Day Support Services provide structured recreation, socialization, education, exercises, and a nutritional meal to dependent adults who live in the community and whose caregivers need respite

Goals and Objectives

The Day Program Goals support clients and family/caregivers in maintaining and promoting a state of wellness, including mental, physical, and emotional health and the prevention of disease and injury. They are:

  • To improve and maintain individual levels of wellness by focusing on physical, social, mental, emotional, and spiritual functioning.

  • To provide services which assist individuals to maintain their independence for as long as possible. To encourage, support, and maximize client’s independence and wellbeing.

  • To provide leisure opportunities and health education; to provide an opportunity for the client to be engaged in his/her own community.

  • To support clients and family/caregivers so they can continue to live in their own homes

  • To provide services, within the continuum of continuing care, which complement and maximize the use of Homecare and other community service providers.

  • To assist clients/family/caregivers in adjusting to changes in life circumstances.

  • To assist family/caregivers in the maintenance of his/her own physical, mental, emotional, and spiritual strength.

  • To delay admission and or ease the transition of a client to a supportive living environment.

  • To facilitate discharge from long term care/rehabilitation/acute care hospitals.
